Don Klitgaard, MD, graduated from the University of Iowa School of Medicine in 1996 and completed his residency at the LMEF family medicine residency program in Lincoln, Nebraska, in 1999. He is the medical director of the Myrtue Medical Center clinics in Harlan, Iowa, where he has been the physician champion as this medical practice participated in the TransforMED National Demonstration Project. This two-year project, sponsored by the American Academy of Family Practice (AAFP), was designed to support and study the implementation of the medical home model of care in real-world practice settings. Dr Klitgaard’s practice is actively evolving into a patient-centered medical home, already having implemented advanced-access scheduling, an electronic health records system with e-prescribing, many office process redesigns, and chronic disease management with in-office health coaches and disease registries. Dr. Klitgaard enjoys practicing full-scope primary care medicine including outpatient, inpatient, emergency, obstetrics, nursing home, and procedural services. Dr Klitgaard has served as the new physician member on the AAFP Board of Directors and is currently president of the Iowa Academy of Family Physicians.
